Effect of Smoking on Early Complications After Elective Orthopaedic Surgery

Cigarette smoking is considered to be a significant risk factor for pulmonary complications following surgery. Tobacco use has also been implicated as a risk factor for delayed fracture healing, failed spine and hindfoot fusions, and increased wound-healing problems. In this study, 811 consecutive patients who underwent hip or knee arthroplasty were studied. Covariates were current smoking and drinking habits, medical history, anesthetic, and type and duration of surgery. The outcome variable studied was the development of any postoperative complication occurring before hospital discharge.Overall, just over one quarter of the patients (28.6%) smoked, and smoking was demonstrated to be the most important risk factor for the development of postoperative complications. Wound-healing problems, cardiopulmonary complications, and the need for postoperative intensive care were the variables most strongly associated with smoking. Not surprisingly, an increased duration of hospitalization was seen for those patients who suffered a complication.This study provides important information for doctors and patients regarding the risk of complications in patients who smoke and who are considering hip or knee replacement. However, more research needs to be done to determine the period of abstinence necessary before arthroplasty to mitigate the increased risk. Patients who smoke should be encouraged to quit, and should be told that they face an increased risk of postoperative complications if they continue to smoke.
