Elbow Instability Management of Elbow InstabilityOperative tactics. The goal in the treatment of fracture-dislocation of the elbow is to restore sufficient stability to the elbow to safely allow early active mobilization. This can usually be achieved by providing stable anatomic fixation of all osseous/articular elements of the injury, thereby reducing the injury to a capsuloligamentous disruption resembling a simple posterior dislocation. When repair of the osseous/articular elements alone does not restore stability, the possibility that a soft tissue structure or osteocartilaginous fragment is trapped within the joint should be considered. Repair or reconstruction of the collateral ligaments should then be considered.
